Patient Information

Please bring the following to your appointment:

  • Your insurance card
  • A list of current medications including over the counter and vitamins
  • A description of the problem, how long you have had it and how it affects you
  • Previous Medical Records

Items to note before your appointment:

You must be registered for each clinic visit or before having a test. Having the appropriate forms filled out before your visit will reduce your total visit time.
Please allow for time to park; parking availability varies with each clinic location.

Transportation:

CHC offers transportation options to and from the clinic. Please call 1 (877) 743-3242 for more information.

After-Hours Care:

During hours when the clinics are closed, our healthcare providers are readily available by telephone via out 24-hour answering service to assist with emergency and critical care situations.

Laboratory and Diagnostic Tests:

CHC has a draw station at most clinic locations. Please visit the clinic locations page to see if a clinic location near you offers lab draw services. Our preferred partner for lab and diagnostic testing is Community Health Clinical Laboratory.

Prescriptions and your Preferred Pharmacy:

  • Please be sure to have a pharmacy location in mind for any prescriptions that may be prescribed during the visit.
  • Refills requests should be called into the original pharmacy where the medication was prescribed to.
  • If you have questions about a new prescription or about discontinuing medications, you can call to speak to your Care Team.
