Back To School Vaccine Clinic 8/12 – Santa Maria

CHC is partnering with Santa Maria School District to offer Immunizations for School Entry*
No appointment necessary! Limited quantities available! We will be offering the following vaccines:
DTAP, MMR, VARICELLA, HEP.B, POLIO & TDAP
Parents must be present and need to bring their child’s vaccine record on the day of service.
